Requesting MonsterCPAP to sign Insurance forms
Select insurance providers prefer patients to use CPAP providers who offer CPAP monitoring and generate compliance reports regularly. As an online vendor, MonsterCPAP does not provide any of these services. Because of this, we decline to sign your Insurance form confirming we offer those services. Visit a CPAP clinic that can provide those services and they can sign the forms accordingly.
